- Have you been a lawful permanent resident (LPR) for at least 5 years (or 3 years if you are married to a U.S. Citizen)?
- Can you speak, read, and write simple English? (Some people are eligible for an exemption).

- Your passport, LPR card, social security card
- Your current and expired passports with dates in which you traveled (last 5 years)
- Your address, work, and school information (last 5 years)
- Your tax returns (last 5 years)
- All of your children’s current address and date of birth
- Spouse’s information (marriage and previous marriage documents)
- IF YOU HAVE HAD ANY CONTACT WITH LAW ENFORCEMENT: Please bring DMV report, LiveScan, and Court Docket
- IF APPLYING FOR A FEE WAIVER: Please being benefits letter and/or copy of your most recent tax return
